Terms: The 5% discount code is redeemable after filling out the Customer Satisfaction Survey.  To redeem your discount code, call 239-481-7725 after submitting the form.  You will be provided with the discount code, once your completed form has been verified.  The discount code is ONLY good for thirty (30) days after the date of issuance and must be used within that time period.  It will expire after the 30 day period and will not be valid after that.  The discount code is only good for a single purchase event.  There is no limit to the amount of products purchased in that event. You are only allowed one (1) customer survey discount code per year, regardless of the amount of times you may submit a Customer Satisfaction Survey.
